    The search fields presented are not case sensitive. Punctuation between search words is not required. For additional information on any of the search fields, click on the question mark (?) icon.

    Use this search method to conduct a more focused search by entering information into any combination of the 15 specific searchable element fields. Information must be entered in the format indicated. The more information entered, the more selective the search results. Using a lesser number of search fields will almost always return more records.

    All of the search fields are joined together by the 'AND' function, which means that the entire search criteria entered has to be contained in each record returned. Some examples:

    By entering: Last name = 'Miller” AND City = 'Evanston' AND State = 'IL' all these elements must be present in order to return a result

    By entering: Last Name = 'Miller' and First Name = 'Steve” will not return a record for ‘Stephen Miller’

    By entering: Last Name = ‘Miller’ and First Name = ‘Stephen’ will not return a record for ‘Steve Miller'

    By entering: Last Name = ‘Miller’ and First Name = ‘Ste’ will return results for both ‘Stephen Miller’ and ‘Steve Miller’

    If the desired result is not returned, try reducing the number of searchable elements or shortening the search parameter (such as from Steve or Stephen to “Ste” per the example above).

    Descriptions for Each of the Searchable Fields

    ABMSUID
    The ABMSUID is a unique identifying number that is assigned by ABMS to each certified physician and is used to verify and efficiently access a particular physician record. ABMSUID is searchable field only on CertiFACTS Online and Certification Profile Service.

    Individual NPI
    A National Provider Identifier or NPI is a unique 10-digit identification number issued to health care providers in the United States by the Centers for Medicare and Medicaid Services (CMS). NPI can be used as a searchable element, but will not be displayed on the results returned. Not all physician records contain a NPI. Individual NPI is searchable field only on CertiFACTS Online and Certification Profile Service.

    Active License Number
    A medical license number is a number issued by a specified government-approved professional association or a government agency are authorized to practice medicine. Active Medical license number is searchable field only on CertiFACTS Online and Certification Profile Service.

    State of Licensure
    A State of Licensure is a state who issued a medical license. State of Licensure is searchable field only on CertiFACTS Online and Certification Profile Service.

    Last Name
    The Last Name field corresponds to the physician’s last name as listed on his or her certificate or congratulatory letter. When entering last names beginning with "De" or "Di" (example: DeSilva), do not place a "space" between the "De" or "Di" (example: De Silva). If the physician uses two last names, (example: Wendell Smith), then enter the identical information into the last name search fields. If a name has changed because of personal circumstances or Anglicization, try refining the search with other information such as date of birth, city and state. The search will only match the names entered to the exact spelling on a physician’s certificate or congratulatory letter.

    First Name
    The First Name field corresponds to the physician’s first name as listed on his or her certificate or congratulatory letter. The search will match the names entered to the exact spelling on a physician’s certificate or congratulatory letter.

    Middle Name
    The Middle Name field corresponds to the physician’s middle name as listed on his or her certificate or congratulatory letter. Not all physician records include a middle name. The search will only match the names entered to the exact spelling on a physician’s certificate or congratulatory letter.

    Member Board
    The Member Board field contains a list of the ABMS Member Boards that award certification. More than one ABMS Member Board name can be selected/deselected by clicking the first selection, then holding down the CRTL key on the keyboard and then selecting/deselecting another ABMS Member Board name. An ABMS Member Board must be selected before selecting a Certification. Member Board is searchable field only on CertiFACTS Online and Certification Profile Service.

    Certification
    The Certification field contains a list of certifications that are available from the ABMS Member Boards. More than one certification can be selected/deselected by clicking the first selection, then holding down the CRTL key on the keyboard and selecting/deselecting another certification. There is no limit to the number of certifications users can select. Selection of more than one Certification option is available only on CertiFACTS Online and Certification Profile Service.

    Certification Date
    The Certification Date field refers to the date that a physician was certified or recertified. Searches on this information can be conducted by entering either 1) the month, day and year, 2) the month and year or 3) the year. Certification Date is searchable field only on CertiFACTS Online and Certification Profile Service.

    Expiration Date
    The Expiration Date field refers to the date that a physician’s certification or recertification expires. Searches on this information can be conducted by entering either 1) the month, day and year, 2) the month and year or 3) the year. Expiration Date is searchable field only on CertiFACTS Online and Certification Profile Service.

    Birth Date
    The Birth Date field refers to the date that a candidate or physician was born. Searches on this information can be conducted by entering either 1) the month, day and year, 2) the month and year or 3) the year. BirthDate is searchable field only on CertiFACTS Online and Certification Profile Service.

    Degree Year
    The Degree Year field refers to the year that a physician received his or her degree. This field can be searched on by entering the year in which the degree was conferred. Degree Year is searchable field only on CertiFACTS Online and Certification Profile Service.

    City
    The City field represents the physician’s city of residence or where his or her practice is located. If nothing is entered in this field, then records from all cities will be searched.

    State/Province
    The State/Province field represents the physician’s state or province of residence or where his or her practice is located. If nothing is entered in this field, then records from all states and provinces will be searched. Selection of more than one state/province option is available only on CertiFACTS Online and Certification Profile Service.

    Country
    The City field represents the physician’s country of residence or where his or her practice is located. If nothing is entered in this field, then records from all countries will be searched. Selection of more than one country option is available only on CertiFACTS Online and Certification Profile Service.
